The St. Paul Athletic Department requires that all athletes travel to and from all athletic contests on the team bus. However, we do realize there maybe family emergencies and situations when an exception might need to be made. In this case, this permission form must be completed (i.e. to the coach and athletic director) at least 24 hours in advance. This is the responsibility of the athlete.

I understand that St. Paul Catholic High School Athletic Rules require that athletes ride the buses to and from all athletic events and a departure from this requirement will release St. Paul Catholic H. S. from all liability for any adverse results that may occur.

I agree to release St. Paul Catholic H. S. and its employees and administration from all liability with reference to the above-stated transportation.

This form must be on file in the Athletic Office prior to the dismissal of school on the day of the contest.
